1. Data controller

PMeD is not a hospital, laboratory or medical decision-maker. It communicates within the scope of consultancy and coordination.
2. Scope and data categories
The general contact form requests your full name, telephone number and email address. When you explicitly submit a Process Assistant request, its service selections, summary and optional coordination message are also sent, together with the source and language. The form sends these details to Formspree to deliver your request to PMeD. No health details or files are requested. Email links open your email application; you send those messages yourself.
Health data, test results, diagnoses, reports, Turkish national identity numbers and other special categories of personal data are not requested through the website. There is no file upload field.
Do not share medical reports, test results, diagnoses or other sensitive health data.
When a page is delivered, connection information such as an IP address, browser information and request time may be transmitted to hosting or external resource servers. These are technical connection data, separate from the contact information requested.
3. Purposes and legal grounds
- Responding to and assessing contact requests and getting back to you: the legitimate interest in managing contact requests under KVKK Article 5(2)(f), provided this does not harm your fundamental rights and freedoms. Article 5(2)(c) applies if a request directly relates to establishing or performing a contract and processing is necessary.
- Information security: Article 5(2)(f) for necessary, proportionate technical operations; Article 5(2)(ç) where a specific legal obligation requires processing.
- Meeting legal obligations: Article 5(2)(ç), limited to the applicable obligation, and Article 5(2)(a) where an operation is expressly provided for by law.
This scope does not include permission for marketing, advertising targeting or processing health data. Reading this notice or clicking an email link does not constitute explicit consent.
4. Collection methods
Contact information is collected electronically through the Formspree contact form or emails you send, by partly or fully automated means. Technical connection data is transmitted during page, form and resource requests. There is no CRM, advertising or analytics integration.
5. Recipient groups and disclosure purposes
Formspree receives the submitted contact details to process and deliver requests to PMeD. Formspree infrastructure may involve servers abroad; applicable KVKK Article 9 transfer requirements must be met. Contact data may be transmitted to the email service provider used to deliver and answer messages. Technical connection data may be transmitted to hosting and external resource providers as needed to deliver the website. Disclosure to authorized public authorities or judicial bodies is limited to legal requirements or duly authorized requests. This contact channel does not transfer client case files to laboratories, physicians or advertising platforms.
The current pages use Google Fonts for fonts and icons and Tailwind CDN for styling. These providers may receive technical connection information. Where servers abroad are involved, a transfer mechanism meeting KVKK Article 9 is required; this notice alone does not authorize a transfer or constitute explicit consent.
6. Retention and deletion
Contact records are kept only for the period necessary to assess and respond to the request and to meet any applicable legal retention obligations. When the purpose and legal necessity end, deletion, destruction or anonymization applies. Indefinite retention or reuse for advertising is not envisaged.
7. Your rights and requests
Under KVKK Article 11, you may ask whether your data is processed, request information about processing, learn the purpose and whether use is consistent with it, learn who receives data in Türkiye or abroad, request correction of incomplete or inaccurate data, and request deletion or destruction where the conditions are met. You may request notification of corrections and deletions to recipients, object to an adverse result arising solely from automated analysis, and seek compensation for damage caused by unlawful processing.

Formal requests may be sent in writing to the business address above or through the methods in the Communiqué on the Principles and Procedures for Requests to the Data Controller. Email requests must meet its conditions, such as using an email address previously provided and recorded. Where needed, a secure method will be determined to verify that a request is yours; the website does not ask you to upload identity or health documents.
Requests are answered as soon as possible and within 30 days. They are normally free; where additional costs arise, the tariff determined by the Personal Data Protection Board may apply.
