1. Who we are and our role
The data controller for the PMeD brand is Ömer Demir (sole proprietorship). Address: Merkez Mah. Abide-i Hürriyet Cad. No: 137, İç Kapı No: 4, Şişli / İstanbul.
PMeD communicates within the scope of consultancy and coordination. It is not a hospital, laboratory or medical decision-maker.
2. Information requested when contacting us
The general contact form requests your full name, telephone number and email address. When you explicitly submit a Process Assistant request, its service selections, summary and optional coordination message are also sent, together with the source and language. The form sends these details to Formspree to deliver your request to PMeD. No health details or files are requested. Email links open your email application; you send those messages yourself.
Do not share medical reports, test results, diagnoses or other sensitive health data.
Do not send Turkish national identity numbers or other special categories of personal data either. This channel is only for a callback request and your contact details. If unnecessary sensitive information arrives inadvertently, it should not be used beyond the contact purpose and should be deleted where there is no need to retain it.
3. Use and retention
Information is used to assess and answer contact requests, get back to you, maintain information security and meet legal obligations. It is not used for advertising targeting, retargeting or marketing profiles. Access should be limited to those who need it for the relevant communication; records are not kept longer than necessary for the purpose and applicable legal obligations.

4. Technical data and external links
Loading the page transmits technical information such as your IP address, browser information and request time to the infrastructure serving the site. The current design loads resources from Google Fonts and Tailwind CDN, which may also receive connection information. This does not mean that an analytics or advertising tool is installed on the site.
When you use map, WhatsApp or email links, the external service's terms and privacy practices also apply. Maps and WhatsApp are external links opened by the user, not embedded tracking tools. Conditions for transfers abroad must be assessed separately for the providers used.
5. Cookies and tracking
The current website application uses no analytics, advertising, retargeting or social media tracking cookies. It does not store visitor profiles in cookies, localStorage or sessionStorage. Process Assistant choices stay in the page's temporary memory until you press Send Request in its own form. Only then are the choices, summary and optional message sent to Formspree. They are not attached to the general contact form. See the Cookie Policy for details.
